Left anterior descending artery length in left and right coronary artery dominance. Estudo com-parativo do padrão de circulação coronariana entre peças anatô micas e pacientes cirúrgicos. A 14-year-old boy was referred to our echocardiography laboratory because of a recent history of palpitations. Both physical examination and ECG were unremarkable. An ambulatory ECG recording detected only rare supraventricular ectopic beats. A two-dimensional transthoracic echocar-diogram (2DE, Panel A, see Supplementary data online, Video S1) raised the suspect of a double-orifice mitral valve (DOMV), without either stenosis or regurgitation at colour and spectral Doppler analyses (Panel B). Three-dimensional transthoracic echocar-diography (3DE) visualization of the valve from both ventricular and atrial perspectives (Panels C and D, see Supplementary data online, Videos S2 and S3) clearly showed the double orifices (white *) with a central bridge of abnormal tissue connecting the anterior with the posterior leaflet. The two orifices were asymmetric, being the medial orifice slightly larger than the lateral one. Valvular leaflets were thin and normally sized. There were two papillary muscles that showed normal morphology and position. No other combined congenital abnormalities were detected. The patient was discharged without performing additional tests and no prescription.

Double orifice mitral valve (DOMV) is an uncommon anomaly that was first described by Greenfield. Since then more than 200 cases have been reported. In this anomaly, mitral valve has a single fibrous annulus which opens with two openings into the left ventricle. We report a case of DOMV accompanied with severe stenosis in a 22-year old female who had complaints of moderate dyspnea. In transth...

Real-time three-dimensional transthoracic echocardiography.

BACKGROUND Complex anatomy of intra-cardiac structures requires spatial orientation of image in three dimensions for better understanding and enhanced image interpretation. We evaluated the feasibility and efficacy of the emerging 'real-time three-dimensional transthoracic echocardiography' technique for comprehensive assessment of cardiac anatomy, physiology, pathomorphology and pathophysiolog...

Analysis of procedural effects of percutaneous edge-to-edge mitral valve repair by 2D and 3D echocardiography.

BACKGROUND Analysis of procedural effects in patients undergoing percutaneous mitral valve repair (PMVR) using the edge-to-edge technique is complex, and common methods to define mitral regurgitation severity based on 2-dimensional (2D) echocardiography are not validated for postprocedural double-orifice mitral valve. This study used 3D transesophageal echocardiography (TEE) to determine the fu...

Double orifice mitral valve with dysplastic tricuspid valve and intact interatrial septum: a three-dimensional echocardiographic study.

Double orifice mitral valve is a rare congenital heart disease. We present a case in which 3D echocardiography helped clarify the underlying anatomy. The need exists for more extensive use of this technique to allow for surgical and pathological confirmation.

Double-orifice mitral valve - echocardiographic findings.

BACKGROUND Double-orifice mitral valve (DOMV) is a rare congenital malformation characterised by the presence of two orifices in the left atrio-ventricular valve area, each having an independent chordal attachment to the papillary muscle. AIM To establish the echocardiographic features and incidence of DOMV in patients undergoing an echo study in a tertiary referral centre. METHODS We carri...
